**Job Summary**: Provide support and care for residential and day care members in their journey towards recovery, community re-integration and an improved quality of life. Coordinate activities for the residents and day care members. Assess members’ needs and design rehabilitation programmes that may include vocational counseling, training and job placement. **Principal Responsibilities**: 1. Caring and providing support for 30 residential / daycare cases. 2. Providing rehabilitative counselling / initiating referrals / organizing family sessions / making home visits for residential / daycare cases when required. 3. Active cases must be seen on a weekly basis. For non-active cases, each case must be reviewed at least once a month and the Review & Admissions Committee once in every three months. 4. Interviewing and making assessments on individuals / families who seek admission to the centre. 5. Consulting and discussing with the Manager when cases involve disciplinary measures and when encountering any difficulties. 6. Consulting and discussing cases recommended for discharge with Review & Admissions Committee, exploring discharge plan and seeking their approval before discharge. 7. Attending Review & Admissions Committee Meetings, Staff Meetings, Case Consultations, Case Conferences, etc. 8. Maintaining documentations and records for services provided to cases. 9. Assisting in other duties and functions assigned by the Manager and/or Centre Director. **Qualification**: 1) Degree in Psychology, Social Work or related discipline. 2) 2-3 years relevant experience. **Core Competencies**: 1) Have a passion to serve persons with mental illness and facilitate their recovery. 2) Active listening skills. 3) Ability to effectively handle crisis situations. 4) Effective skills in oral or written communication. 5) Bilingual (English and Mandarin preferred). 6) Ability to co-operate and work well within the team as well as independently.
