Job Description

We are seeking a compassionate and skilled caregiver to provide high-quality care to chronically ill patients in their homes.

The ideal candidate will have experience working with palliative care patients, conducting home visits, performing comprehensive nursing assessments, and coordinating with doctors for medical reviews. They will also be responsible for monitoring medication compliance, supporting prescribed care plans, and carrying out nursing procedures such as wound care and insertion of urinary catheter.

Responsibilities:

  • Conduct home visits for chronically ill patients
  • Perform comprehensive nursing assessments and coordinate with doctors for medical reviews
  • Monitor medication compliance and support prescribed care plans
  • Carry out nursing procedures including wound care, insertion of urinary catheter, and general tubing care

Required Skills and Qualifications

To be successful in this role, the ideal candidate will possess the following skills and qualifications:

  • Valid Singapore Nursing Board (SNB) registration
  • Current BCLS certification
  • Palliative care experience
